Simply download and read the Product Disclosure Statement (PDS) and complete the following forms as required:

• Application Form
• Investment Options PDS
• Binding Nomination Form (Optional)
• Tax File Number Notification Form (Fill out to avoid unnecessary taxation)
• Benefit Transfer Authority to transfer funds from another superannuation account
