Consolidate my super?

To consolidate your existing super acounts into your EquitySuper account, request or download a Benefit Transfer Authority form, complete it and return it to Equity Trustees Superannuation Limited at Reply Paid, Box 398, North Sydney NSW 2059.

Most Superannuation Providers have additional requirements for transfers of super funds (e.g. certified proof of identity documents). You may need to contact your other Superannuation Provider/s to determine these requirements, or they will contact you after receiving the completed Benefit Transfer Authority form.
